Smith suffered the injury in a Sept. 30 game against Seattle, and was trying to come back this season. However, with the 49ers at 3-10 this season and after Smith met with doctors Monday at Stanford Hospital and consulted by phone with noted sports surgeon Dr. James Andrews, the QB then apparently decided to shut it down, according to the San Francisco Chronicle. Also, Trent Dilfer is considered doubtful to play in Saturday's game against Cincinnati due to a concussion. Dilfer left Sunday's 27-7 loss to the Minnesota Vikings with 19 seconds left in the second quarter with the injury. On 4th-and-2 at the Vikings' nine-yard line, Dilfer was flushed from the pocket and scrambled to his left. While stumbling short of the marker, Dilfer was nailed in the head by Charles Gordon and laid motionless for several minutes. Dilfer eventually sat up and was assisted on the field, and then was taken by cart to the locker room. He went to a local hospital, but was released Sunday night. It now seems Shaun Hill will get the start at quarterback this week.
